Begin your fertility journey with a clinician at Sesame.
To qualify for program savings, you must be a Costco Member, a Sesame subscriber, and a new Everleap Fertility patient. Your Sesame visit is the required first step and includes your initial consultation, health history review, diagnostic coordination, and transparent self-pay pricing information.
Receive Your Referral
Connect with a fertility specialist through the program.
If advanced fertility treatment is recommended, your Sesame clinician will provide a referral to an IVI RMA fertility specialist. This coordinated referral pathway ensures a seamless transition to expert care and is required to access eligible program discounts.

Move forward with treatment and medication savings.
Once referred through the program, you'll receive discounted self-pay pricing on eligible fertility services at participating IVI RMA clinics, along with access to Costco Pharmacy pricing that can save you up to 80% on fertility medications.

To qualify, you must be a new Everleap Fertility patient, an active Costco member, a Sesame subscriber, and receive a referral from a Sesame clinician.

Eligibility requires an active Costco membership and starting care with a Sesame clinician. The initial Sesame visit covers intake, diagnostic coordination, and transparent self‑pay pricing, and is required to access program benefits.<br>
Seamless Referral to Everleap Fertility
After intake, Sesame facilitates a streamlined referral to a Everleap Fertility specialist for advanced treatment when appropriate. To receive the discount and coordinated support, patients must enter care through the Sesame referral pathway.
Save on Treatment and Fertility Meds
Once referred, you can access savings on self‑pay treatment at Conceptions and save up to 80% on fertility medications through Costco Pharmacy’s exclusive pricing for members.
This program is not available to current Everleap Fertility patients, Costco members who do not use Sesame, or patients referred by providers outside the Sesame platform.

Everleap Fertility in Colorado has offices in Littleton, Denver, Lafayette, and Lone Tree.
If you’re not near a participating clinic, you can still enroll. Our telehealth services and remote monitoring allow patients to access the program from anywhere in the U.S.
What the Treatment Discounts Cover
These savings help lower the cost of essential fertility services for self‑pay patients.
Here are the services the self‑pay discount covers:
- Intrauterine insemination (IUI)
- In vitro fertilization (IVF)

- Egg freezing

The discount excludes fertility medications, PGT/genetic testing, anesthesia, donor eggs, donor sperm, surrogacy, outside labs, and any services not billed directly by Everleap Fertility.

Excludes services not billed directly by Everleap Fertility, like anesthesia, donor eggs, donor sperm, and surrogacy.
- Patients must choose self-pay to access the Everleap Fertility discounted pricing. Discounted services cannot be billed to insurance.
- Individuals receive a discount on Costco Specialty Pharmacy of 80% off
Does the discount apply to treatment packages?
- Yes. At participating clinics, the Everleap Fertility discount applies to the full package price
- If the clinic does not participate in treatment packages, the discount applies to all eligible self-pay fee-for-service charges instead
- Packages are standardized, not customizable or prorated
